The Ohio Department of Medicaid (ODM) is committed to improving the health outcomes for Ohioans through innovation. To achieve this, ODM is sponsoring quality improvement partnerships across Ohio.
The University of Cincinnati (UC) is honored to share in this collaboration with practices in our region, as well as with patients and community organizations. By forming an improvement science learning network, our UC Hub aims to support primary care practices in implementing known best practices as well as in discovering and co creating new processes to address equitable health outcomes.
Topic of Focus: Hypertension
Cardiovascular (CV) disease, which includes heart disease, stroke, and heart failure, is the leading cause of death in Ohio, and the US. (CDC, 2018 Vital Statistics).
Hypertension is also a major cause of end stage kidney disease requiring dialysis.
Hypertension is the number #1 cause of cardiovascular disease.
Hypertension remains a persistent public health crisis.
